Responsibilities

Assesses patient's status through interview, observation and physical assessment.
Develops individualized plan of care with patient/family and health care team.
Uses interventions identified in the plan to achieve outcomes.
Evaluates and documents patient progress on an ongoing basis.
Modifies plan of care based on evaluation and changes in patient condition.
Completes required documentation including patient chart and event reports.
Demonstrates accountability for own practice and continuous learning.
Troubleshoots equipment and maintains current knowledge in evidence-based nursing.
Promotes patient and staff safety with proper use of lift equipment.
