Qualifications

• BSN required as of the last update (4/27/2025).

Responsibilities

• Assess patients' physical, social, and psychological status and gather information from the care team.
• Plan and develop an individualized care plan with patient and family involvement.
• Coordinate care and educate patients, families, and other teams.
• Direct and delegate care, ensuring timely documentation and adherence to guidelines.
• Evaluate outcomes and adjust the care plan as needed.
