Thresholds-Inc in Illinois seeks a Crisis Worker to provide frontline behavioral health crisis intervention in the hospital ER. You will perform risk assessments, determine disposition, coordinate referrals, and support patients through stabilization and care planning.

The role requires a Master's degree in counseling or social work and LPHA licensure is preferred. Expect collaboration with medical teams and robust documentation in SmartCare and Cerner; part-time weekend schedule is specified.

Qualifications

• Master's degree in counseling or social work or closely related field.
• LPHA license (LCSW or LCPC) preferred.
• Experience working with persons diagnosed with mental illness preferred.
• Knowledge of medical terms and ability to communicate with medical staff is strongly preferred.

Responsabilités

• Performs risk assessment in an emergency room setting to determine disposition and recommendations for treatment.
• Use clinical judgment to make quick decisions in high-stress situations with high patient volume.
• Complete documentation to facilitate patient transfer to inpatient facilities (USARF and Petition).
• Collaborate with ER physicians, nurses, technicians, and psychiatry staff.
• Use de-escalation skills to assist in crisis interventions.
• Provide psychoeducation and bedside interventions to ED patients.
• Assist inpatient psychiatry on medical floors with assessments and linkage to care.
• Contact resources, hospitals, and agencies to link patients to additional care.
• Gather collateral information to assess service needs and identify supports.
• May communicate significant information to leadership; provide timely updates to supervisor.
• Meet billing targets and complete documentation in SmartCare and Cerner.
• Other duties as assigned.
• Shift Schedule: Part Time - Saturday & Sunday, 2:45pm - 11:15pm.
