This position is remote within the state of AZ only. This remote work opportunity requires residency and work to be performed within the State of Arizona. Depending on business needs may need to travel to the office.

Purpose of the job

The Health Promotion Coordinator is responsible for tasks related to increasing compliance with performance measures and promoting comprehensive health care for members from birth to age 21.

ESSENTIAL job functions AND RESPONSIBILITIES

  • Enter information from the tracking forms into the Medical Management System
  • Process Referrals
  • Forward specific referrals to appropriate departments
  • Maintain database and documentation
  • Enter and update data in the Immunization database
  • Outreach made to the parent / guardian of members whose immunizations are not up-to-date
  • Contact members via telephone and / or mail on child health and wellness related topics
  • Communicate with providers on member related issues
  • Follow up calls are made to members who do not show to their scheduled doctor visit to identify possible obstacles to attending scheduled visits
  • Outreach to members referred to services to verify that needs have been met
  • Perform all other duties as assigned
  • The position requires a full-time work schedule. Full-time is defined as working at least 40 hours per week, plus any additional hours as requested or as needed to meet business requirements

REQUIRED COMPETENCIES

Required Job Skills

  • Knowledge of Medicaid regulations and guidelines
  • Knowledge of childhood immunization requirements
  • Familiarity with medical terminology
  • Computer experience in Microsoft Excel, Word, Outlook
  • Problem solving abilities
  • Ability to prioritize work tasks to adhere to deadlines and identified time frames
  • Typing 35-40 wpm
