e4health is seeking an experienced Epic Healthy Planet / Value-Based Analytics Consultant to optimize population health, quality performance, and clinical outcomes across healthcare organizations. You will translate requirements into analytics, dashboards, and reporting solutions, collaborating with clinical, operational, and IT teams to impact care delivery and cost of care.

Qualifications

5+ years in healthcare IT, clinical informatics, population health, or analytics.
At least 5 years of demonstrated Epic Healthy Planet project work.
Experience supporting Epic Healthy Planet implementations or major optimizations.
Strong understanding of value-based care and population health analytics.
Ability to translate requirements into analytical and reporting solutions.
Knowledge of healthcare quality programs and performance measures.
Excellent communication with clinical and IT stakeholders.

Responsibilities

Provide functional and analytical support for Epic Healthy Planet implementations and optimization.
Translate value-based care requirements into workflows, reporting, and analytics.
Develop dashboards and reports for population health, CMS, HEDIS, MIPS, and Medicare programs.
Collaborate with clinical, quality, finance, and IT teams to drive interventions.
Validate data, test analytics, and troubleshoot Healthy Planet reporting.
Support risk stratification and care gap identification using registry data.
Contribute to executive-level reporting and performance scorecards.
