Job Overview: The EPC Psych Case Manager position will receive referrals from both inpatient and outpatient services. Clients can be either EPC or voluntary referrals. This person will be responsible to perform an Intake Assessment and Individualized Program Plan for each client. Provide on‑going support, advocacy, and crisis management follow‑up services for referred patients. Length of service may be up to six months in duration.
Bachelor’s Degree in a Mental Health related field (Social Work, Psychology, etc.) required.
