Job summary

Qualifications

• Minimum associate's degree or equivalent in social work, psychology, or gerontology preferred.
• Two or more years of experience in social service delivery with the elderly and/or family populations is a plus.
• Demonstrated ability to advocate, organize, problem-solve, and provide results for residents served.

Responsibilities

• Coordinate programs and services to help residents maintain a good quality of life.
• Assess and identify resident and family needs and outside resources.
• Assist in coordinating home care services to enable independent living.
