Job summary

Proforma Health Inc. seeks an Otolaryngology Physician to deliver patient-centered care within a vibrant Lakeland community. You will provide clinic-based ENT services in a Monday–Friday, 8:00am–5:00pm schedule with hospital call rotations.

Qualifications

• Board-certified or board-eligible Otolaryngology (ABMS/AOA).
• Florida medical license or ability to obtain one.
• Commitment to patient-centered care and collaborative practice.

Responsibilities

• Provide patient-centered otolaryngology care in clinic and hospital settings.
• Clinic 8:00 AM–5:00 PM, Monday–Friday with hospital call rotations.
• Collaborate with ENT APRNs for inpatient consults and ensure continuity of care.
• Utilize diagnostic tools such as microscopes, fiber optic laryngoscopes, and sinus scopes; onsite allergy testing.

Education

Board certification or eligibility via ABMS or AOA

Florida medical license or ability to obtain one

• Support includes a dedicated Pediatric OR, ICU, and ED.
• Be part of a large physician group with over 400 providers across more than 40 specialties.
