Job summary

Azza HealthCare Agency in Danville, PA is seeking a dedicated care coordinator to support patients entering care programs. You will act as a liaison between patients and healthcare providers, guiding them through the treatment journey and ensuring continuity of care.

You will coordinate pre- and post-hospital care with multidisciplinary teams, educate patients, and reinforce information through clear communication.

Qualifications

• Graduate of a Specialty Training Program (required).
• Bachelor's Degree in Nursing (preferred).
• Strong communication, coordination, and patient education skills.
• Ability to collaborate effectively with multidisciplinary teams.

Responsibilities

• Facilitates entrance of new patients into care programs and ensures continuity of care.
• Coordinates pre- and post-hospital care, and collaborates with healthcare teams to develop and implement patient care plans.
• Provides patient education, promotes understanding of conditions, and reinforces information through various communication methods.
• Supports clinical activities, assists physicians, and educates team members.
• Coordinates patient activities with support services, maintains follow-up systems, and ensures timely surveillance.
• Acts as a resource for patients and team members, and manages interdisciplinary activities to meet patient needs.
