Job description

Care First is seeking an Enrollment Specialist to join our team in The Bronx. This role serves as the first point of contact for prospective participants and is responsible for identifying, engaging, and qualifying eligible individuals through our enrollment and intake process.

The Enrollment Specialist will educate potential participants about available services, assess eligibility based on Medicaid status and social determinants of health criteria, complete intake documentation, and ensure a seamless transition to the appropriate care or support team once eligibility has been confirmed.

The ideal candidate is compassionate, detail-oriented, and skilled at building trust with diverse populations. They are motivated by helping individuals access resources that can improve their health outcomes and quality of life.

Responsibilities
  • Conduct Eligibility Screenings
    • Contact prospective participants via phone, text, email, or community outreach efforts
    • Complete intake assessments to determine eligibility based on Medicaid enrollment and established social determinants of health criteria
    • Accurately collect, verify, and document participant information
  • Guide Participants Through the Enrollment Process
    • Educate individuals about available programs, services, and benefits
    • Assist participants with completing enrollment forms and required documentation
    • Address questions and concerns throughout the intake process
  • Maintain Accurate Records
    • Document all participant interactions and enrollment activities within the CRM and enrollment systems
    • Ensure data integrity, accuracy, and compliance with organizational policies and regulatory requirements
  • Meet Enrollment Goals
    • Achieve individual and team enrollment and qualification targets
    • Maintain consistent follow-up with prospective participants to maximize completed enrollments
  • Collaborate with Internal Teams
    • Coordinate smooth handoffs of qualified participants to care management, case management, or service delivery teams
    • Communicate participant needs and relevant information to support continuity of care and services
  • Community Outreach and Engagement
    • Build relationships with community organizations, healthcare providers, and referral sources
    • Participate in outreach events and initiatives designed to increase awareness and enrollment opportunities
Requirements
  • 1–3 years of experience in enrollment, healthcare outreach, patient access, customer service, intake coordination, Sales, community health, or a related field
  • Experience working with Medicaid populations, health plans, community-based organizations, or social service programs preferred
  • Strong interpersonal and communication skills with the ability to engage individuals from diverse backgrounds
  • Ability to explain complex information clearly and compassionately
  • Excellent organizational skills and attention to detail
  • Experience working with CRM systems, enrollment platforms, and Microsoft Office applications
  • Ability to manage multiple priorities while meeting performance goals
  • High school diploma or equivalent required; associate's or bachelor's degree preferred
