• Previous experience in outreach, community engagement, social services, or related field.

Responsibilities

• Patient Engagement; Build trust and rapport as the first voice patients hear.
• Program Education; Serve as an internal expert on the member onboarding experience.
• Enrollment Support; Manage a call list to educate target members on program benefits.
• Growth Enablement; Drive participation and support organizational enrollment goals.
• Patient Satisfaction; Ensure patients receive efficient care coordination.
• Collaborate with the clinical team in scheduling provider visits.
• Educate patients and families about health conditions and treatment options.
• Advocate for patients within the healthcare system.

Skills

Bilingual: Fluent in Spanish and English

Excellent communication and public speaking skills

Ability to work independently and travel within the Bronx

Strong organizational and time management skills

Cultural sensitivity and ability to work with diverse, underserved populations

Proficiency in motivational interviewing and health coaching techniques

Proficiency in Microsoft suite including Teams and Excel

Education
