An established industry player is seeking a Qualified Mental Health Professional to join their multidisciplinary team. This role involves providing essential behavioral health services to older adults and individuals with disabilities facing serious mental health challenges. Yamhill County has one full-time position available for a Qualified Mental Health Professional (Behavioral Health Clinician or Advanced Practice Behavioral Health Clinician) to provide behavioral health services and program coordination at the Enhanced Care Facility (ECF). The ECF serves older adults and adults with disabilities experiencing serious mental illness and co-occurring medical conditions in a licensed residential setting.
Working within a multidisciplinary team, the role involves delivering person-centered services such as assessment, treatment planning, therapy, behavior support, consultation, and case management to residents. Additional duties include overseeing programming activities, training staff, collaborating with facility administration, screening referrals, discharge planning, and monthly reporting. The caseload is small (16 clients), offering a balance of clinical and administrative responsibilities.
Required: Master’s in Social Work or a related social services field, or a bachelor’s in Occupational Therapy or Nursing from an accredited school. Specific experience requirements depend on the role:
Experience working with older adults with co-occurring mental health and medical conditions is preferred.
The ideal candidate understands community mental health techniques, can develop effective relationships, and maintains detailed documentation. They should be conscientious, collaborative, organized, and able to adhere to policies, with strong communication skills and professionalism.
