Job summary

The Laurels of Galesburg, part of Ciena Healthcare, seeks an Activity Assistant Director to support daily activity programs that promote residents’ physical, mental, and psychosocial well‑being. You will help plan, facilitate, and document activities to create an engaging, community‑minded atmosphere in a compassionate skilled‑nursing setting.

Requirements include a high school diploma or GED, basic computer skills, and the ability to work with residents, families and volunteers.

Qualifications

• Minimum high school diploma or GED.
• Experience in activities, recreation therapy, and long-term care is preferred.
• Computer skills are required.
• Meets state and federal criminal background check requirements.

Responsibilities

• Facilitates activity programs as planned to meet the functional levels, needs and interests of each Resident.
• Assist Resident to and from activity programs and outings as scheduled, as required.
• Maintains good rapport with family members and encourages family support in programs.
• Completes appropriate records indicating Resident’s attendance and participation in individual and group activities in a timely basis.
• Provide input to the assessment and evaluation of Residents' participation when requested. Communicates Resident’s concerns and responses to interventions to immediate supervisor.
• Supports Residents' individual needs, preferences, customary routines, concerns and choices.
• Assists with escorting Residents to and from activities.
• Assist and work with individual volunteers and volunteer programs at the request of the Activity Director.
• Cleans equipment and materials used for activities.
• Participates in long-term care survey process, regulatory, and accreditation requirements.
• Performs other duties as assigned.
