• Utilizes theoretical and evidence-based knowledge in partnering with patients and their families to assess, plan, implement and evaluate age appropriate care.
• Assesses and collaborates with patient, family/significant others and other disciplines to identify expected outcomes and develop a care plan.
• Plans and develops individualized outcomes-based care plans and orchestrates the plan of care for each shift; coordinates discharge planning with resources.
