Infinite Medical P.C. is a nationwide network of advanced practice providers and specialty clinicians. We partner with The MedElite Group to place Endocrinology Nurse Practitioners and Physician Assistants in skilled nursing facilities across New York.

We seek a friendly, compassionate NP/PA to provide comprehensive endocrine care, including diabetes and thyroid management. Travel to multiple nursing homes within the state is required; a Master’s in Nursing and an active NP/PA license are

Qualifications

• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License
• CPR BCLS/BLS certified
• Must hold a Master’s Degree in Nursing
• Completed a degree from an accredited Nurse Practitioner program
• A year of experience in Endocrinology
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
• Must have great organizational and time management skills.
• Reliable transportation
• Must be willing to travel to different nursing homes within the state of Florida (100% commitment)

Responsibilities

• Assist care coordinators across the different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patients with Endo conditions and report through our proprietary software system
• You will be cross-trained in other areas of care management
• Work under the supervision of an off-site Collaborative Physician
• Gathering and analyzing patient information
• Detailed review of the system
• Review of all pertinent patient labs, studies, and provider notes
