Job summary

Infinite Medical P.C. seeks an experienced Endocrinology Nurse Practitioner or Physician Assistant to join a travel-ready team.

You will provide endocrine care for residents across multiple facilities in New York, manage diabetes and thyroid conditions, and collaborate with off-site physicians using our care-management system. The role requires an NP/PA license, an active DEA, at least 6 months in long-term care, and a Master’s in Nursing.

Qualifications

• Active NP/PA license and DEA license required.
• Minimum 6 months experience in long-term care or skilled nursing facility.
• Masters in Nursing from an accredited program.

Responsibilities

• Assist care coordinators across providers and settings.
• Advance patient outcomes via communication between facilities and patients.
• Assess endocrine patients and document findings in the EMR.
• Cross-train in other care-management areas.
• Work under supervision of an off-site Collaborative Physician.
• Gather and analyze patient information for care planning.
• Review patient labs, studies, and provider notes.

Job Summary

As an Endocrinology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues. Responsibilities

• Assist care coordinators across the different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patients with Endo conditions and report through our proprietary software system
• You will be cross-trained in other areas of care management
• Work under the supervision of an off-site Collaborative Physician
• Gathering and analyzing patient information
• Detailed review of the system
• Review of all pertinent patient labs, studies, and provider notes

Requirements

• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• DEA License
• CPR BCLS/BLS certified
• Must hold a Master's Degree in Nursing
• Completed a degree from an accredited Nurse Practitioner program
• A year of experience in Endocrinology
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
• Must have great organizational and time management skills.
• Reliable transportation
• Must be willing to travel to different nursing homes within the state of Florida (100% commitment)
