Job summary

Infinite Medical P.C. is seeking a friendly Endocrinology Nurse Practitioner/Physician Assistant to join our team. This part‑time role (3 days per week) focuses on comprehensive endocrine care, including diabetes and thyroid conditions, with a collaborative, patient‑centred approach.

Qualifications

• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License.
• CPR/BLS certification.
• Must hold a Master's Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• A year of experience in Endocrinology.
• Experience documenting EMR preferred (Sigma, Visual, PCC) training provided.
• Must have strong organizational and time-management skills.
• Must be able to lift at least 20 pounds and stand/walk/work up to 6–8 hours per day.

Responsibilities

• Provide comprehensive healthcare services and coordinate patient care.
• Assess and treat endocrinology diseases and conditions.
• Work closely with a team of healthcare professionals.
• Assess, diagnose, and treat residents' physical and mental well-being.
• Conduct thorough patient assessments, including medical history and physical exams.
• Order and interpret diagnostic tests.
• Develop accurate diagnoses and treatment plans.
• Prescribe medications and administer treatments.
• Provide counseling to residents and their families.
• Collaborate with the interdisciplinary team for seamless coordination of care.
• Manage chronic conditions and monitor progress.
• Make appropriate adjustments to care plans.
• Commit to person-centered care.
• Create a safe and nurturing environment for elderly residents in the nursing home.
