Job summary

Healthcare United is seeking an experienced Endocrinology Nurse Practitioner to join an outpatient specialty practice in Southern Connecticut. The role requires at least three years of provider experience and an active or eligible Connecticut APRN license.

You will collaborate with endocrinologists, APPs, and clinical staff to deliver comprehensive care for diabetes management, thyroid disease, and metabolic disorders in a busy outpatient setting.

Qualifications

• Active or eligible Connecticut APRN license.
• Minimum of three years of provider experience.
• Endocrinology or diabetes management experience preferred.
• Strong clinical assessment, communication, and patient education skills.

Responsibilities

• Evaluate and manage endocrinology patients
• Provide care for diabetes, thyroid disease, metabolic disorders, and related conditions
• Conduct follow-up visits and patient assessments
• Order and interpret diagnostic testing
• Develop and adjust treatment plans in collaboration with physicians
• Educate patients on medications, lifestyle changes, and disease management
• Maintain accurate EMR documentation
• Support continuity of care within the endocrinology program
