Job summary

Hackensack Meridian Health's endocrinology team is seeking a Clinical Coordinator (RN) to identify, support and develop programs for complex patients and families. Working with the Clinical Program Manager and subspecialty clinicians, you will coordinate care and provide education across the patient journey.

This busy practice supports five endocrinologists, with a focus on diabetes management, and requires BSN, 3–5 years of hands-on care, and strong communication.

Qualifications

• RN with clinical expertise in coordinating care across subspecialties.
• Strong communication with patients, families, and multidisciplinary teams.
• Proficient computer skills (MS Office/Google Suite).

Responsibilities

• Maintains a communication system ensuring timely information flow to providers and team.
• Interacts with patients and families, assesses needs, and supports care planning.
• Documents interventions and plans in the EMR with accuracy.
• Collaborates with the multidisciplinary team on care planning and evaluation.
• Facilitates tests, prescriptions, and provider communications as needed.
• Acts as a clinical resource and participates in guideline development.
