Qualifications

• Endocrinologist with medical license in Illinois.
• Board certification in Endocrinology is preferred.
• Experience in outpatient endocrinology is a plus.

Responsibilities

• Provide endocrine care to adult patients within a collaborative network.
• Collaborate with PCPs and other specialists to coordinate comprehensive care.
• Deliver evidence-based, patient-centered treatment plans and follow-up.
