Job summary

Horizon Services Inc. is seeking an EMT – Health Tech I/II in San Leandro, CA. This full-time position monitors vital signs and withdrawal risks, administers medications, and coordinates care with medical and detox staff.

Qualified candidates are EMT certified with CA license; CPR/First Aid; and a willingness to work on a shift-based schedule in a sobering center setting. Bilingual skills are a plus.

Responsibilities

We are seeking a Health Tech who is passionate about serving diverse communities. Bilingual candidates fluent in one or more of BHSD’s threshold languages—Chinese, Spanish, Tagalog, or Vietnamese—are eligible for an additional incentive.

As a Health Technician I

As a Health Technician I, you provide a safe environment to participants by monitoring potential medical and mental health issues as the participant goes through early stages of withdrawal on an on call and or as needed basis. Horizon Service, Inc. – Cherry Hill Sobering Centers, located in San Leandro, CA is a sobering center supporting participants who arrive at the center for public intoxication as an alternative to incarceration and unnecessary hospitalization. Cherry Hill serves as the primary entry point for substance users in Alameda County. You will connect with participants through hourly monitoring of vitals, making medications available, collaborating with participant’s medical/mental health providers, performing withdrawal assessments and responding to program emergencies.

Responsibilities

• Perform health screening for all potential clients.
• Monitor vitals, medical triage, and assess clients’ needs for sobering.
• Maintain records of client’s medication in logbook and EHR system (if applicable).
• Perform an appropriate assessment which may include rapid risk intoxication assessment and vitals for every client entering the sobering center.
• Document observations 30 min
• Case notes every 6 hours (in EHR if applicable) which includes medications observation, case management, crisis interventions and discharge
• Create a welcoming environment for participants.
• Perform conflict resolution and de‑escalation of hostile client.
• Respond to crises, emergencies and other client needs in an appropriate and timely manner.
• Provide Transportation as necessary and when requested.

Team Support

• Work collaboratively as part of the multi-disciplinary team at the Recovery Station.
• Daily monitoring and maintenance of client files (EHR if applicable) for errors, corrections, and updating.
• Participate in daily shift change and report health related concerns and alerts.
• Collaborate with Lead Nurse Coordinator and or LVN to reduce excessive Emergency room visits for medication clearances.
• Support in keeping a clean and safe environment.

Quality Assurance Support

• Daily monitoring and maintenance of client files (EHR if applicable) for errors, corrections, and updating.
• Ensure all completed assessments are placed in file and or scanned and uploaded into the client chart daily.
• Ensure all medication clearances and Emergency Room discharge paperwork is placed in file and or scanned and uploaded daily into the client chart.
• Document all medical concerns and observations in client file (EHR if applicable) and give verbal report to Detox staff upon transferring a client to Detox from Sobering if applicable.

Qualifications and Requirements

• Certified Emergency Medical Technician EMT with the State of California
• Basic academic knowledge and understanding of Substance Use Disorders and Mental Health
• CPR/First Aid certified.
• Must have a CA driver’s license and good driving record in good standing
• Background and drug testing at time of hire
• Current TB test and retaken annually
• Preferred: Bilingual in English, Spanish, Chinese, Vietnamese, or Tagalog
