Job summary

Limitless Possibilities LLC is seeking direct support professionals to supervise adults with disabilities in homes, communities, and vocational sites. You will address each client's needs, teach opportunities, offer informed choices, and follow service plans under supervision.

Responsibilities include household management, service plan facilitation, health promotion, documentation, advocacy, and client empowerment. The role requires lifting up to 25 pounds and strong communication skills.

Qualifications

• Be at least 18 years of age (21 for driving positions)
• Possess at least a high school diploma or equivalent
• Some experience in providing community supports to people with developmental disabilities
• Ability to reason and problem solve
• Strong written and verbal communication skills
• Possess a valid driver license
• Possess automobile insurance
• Willingness to perform any reasonable request to meet clients' needs
• Ability to use computer applications such as word processor, e-mail, etc.
• Capable of working with adults with developmental disabilities through verbal instruction, demonstration, training and cooperation
• Stand, sit, bend, lift, climb, balance, stoop, kneel, crouch and move intermittently
• Must frequently lift/move up to 25 pounds

Responsibilities

• Household Management: assist with meal prep, laundry, cleaning and decorating and transportation needs to maximize independence.
• Facilitation of Services: fulfill roles related to individual service plan development, implementation, and review.

• Documentation: manage documentation requirements efficiently.
• Client Empowerment: support self-determination, self-esteem and decision-making.
• Assessment: use formal and informal assessment practices to respond to needs, desires and interests.
• Advocacy: be knowledgeable about diverse challenges facing individuals.
• Community and Service Networking: identify and access supports in the community.
• Building and Maintaining Friendships: support development of friendships and relationships.
• Communication: use effective communication strategies to build collaborative relationships.
