You will be part of a team that includes Day Services Coordinators, Nursing staff, and advocates, delivering person-centered support across daily living activities and community engagement.

Responsibilities include advocacy, documentation, and teaching daily living skills while maintaining safety, privacy, and dignity for each person served.

Responsibilities

• Advocacy: support self-advocacy and help people live self-directed lives with partners and community.
• Documentation: record preferences, dreams, and progress toward goals with education and exposure.
• Self Determination: help people direct their own lives and build self-esteem and decisiveness.
• Communication: relay information timely and accurately per agency policies to the team.
• Outcome Planning: assist to develop and implement plans reflecting the person’s dreams and desires.
