Job summary

Community Options, Inc. seeks a Full-Time Registered Nurse (RN) in Manhattan, NY to deliver home health care to individuals with disabilities, using the nursing process of assessment, planning, implementation, and evaluation.

Starting rate ranges from $90,000 to $110,000 annually, depending on experience; current RN license required with 2+ years of RN experience. OPWDD experience and Diabetes Education Certification preferred.

Qualifications

• Current Nursing License required.
• Minimum 2 years’ RN experience required.
• OPWDD experience preferred.
• Bachelor’s Degree preferred.
• Diabetes Education Certification preferred.

Responsibilities

• Participate in the admission of new residents to assigned homes.
• Develop plans to meet medical and healthcare needs and ensure appropriate medical follow-up.
• Review lab work and coordinate annual physicals with clinics and hospitals.
• Provide instructions to residents and staff on medical diagnoses, allergies, and diet.
• Clarify medication orders and contact physicians to review status.
