Humana's CenterWell Home Health is hiring a Registered Nurse to provide skilled, one-on-one care in patients' homes. Reporting to the Clinical Manager, you will manage care plans, administer medications, perform wound care, and coordinate with physicians and other clinicians to support recovery and independence.

The role requires current RN licensure, CPR, and experience in Med/Surg, ICU, ER, or acute care, plus a valid driver's license and reliable transportation.

Formación

• Med/Surg, ICU, ER, or acute care experience required.
• Current and unrestricted RN licensure.
• Current CPR certification and valid driver's license required.

Responsabilidades

• Provide skilled nursing care in patients' homes.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management.
• Maintain accurate records and coordinate with care team.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure documentation accuracy.
