Greater Hearts Human Services LLC is seeking Direct Support Professionals to assist clients with intellectual and developmental disabilities in a Chester County, PA home/residence. You will implement individual support plans and work under a Team Lead to promote self-directed living and community integration.

Responsibilities include supervising daily activities, health and safety, personal care, transport, and collaboration with families and providers.

Qualifications

• Ability to learn state regulations and pass Medication Administration course
• Ability to foster a respectful home environment and relationships
• Ability to relate to clients with disabilities and assess needs
• Strong communication and writing skills in English

Responsibilities

• Work independently or as part of a team in home/residential settings
• Develop and implement client support plans
• Ensure health, safety, and welfare; provide supervision as per plan
• Assist clients in daily living skills and community integration
• Transport clients to appointments and activities as needed
• Provide coverage in emergency situations and participate in meetings
