Job summary

Qualifications

• Licensed Social Worker in New Jersey (LCSW preferred).

• Minimum of 1 year of social work experience (homecare or healthcare exposure preferred).

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Coordinate care with nurses, therapists, physicians, and community resources.
• Provide counseling, crisis intervention, and education to patients and caregivers.
• Connect patients and families with community resources, financial aid, and support services.
• Maintain accurate documentation to ensure continuity and quality of care.
