Job summary

Humana’s CenterWell Home Health division is seeking a Home Health Registered Nurse to provide one-on-one home care under the Clinical Manager. You’ll manage care plans, educate families, and coordinate with physicians to support recovery and independence for patients in their homes.

The role emphasizes skilled nursing care, medication administration, and effective teamwork with clinicians. A valid RN license, CPR, and a valid driver’s license are required for this patient-facing position.

Qualifications

• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.
• Home health or community nursing experience preferred.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
