Job summary

LiveWell Partners is seeking a compassionate Registered Nurse (RN) Case Manager to work one-on-one with patients in their home environment, developing and overseeing care plans to enhance independence and satisfaction. Collaborate with a supportive team of clinicians and adhere to physician orders to achieve optimal outcomes.

Qualifications

• Minimum of 2 years of nursing experience in acute or post-acute care setting preferred.
• Active State Registered Nurse licensure is required.
• Proof of current CPR certification and valid Driver’s license.

Responsibilities

• Assumes responsibility for assigned patient’s care from admission through discharge by effectively executing patient’s Plan of Care.
• Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
• Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
• Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
• Participate in on-call rotation as reasonably assigned.
