Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one, in-home nursing care under the Clinical Manager. You will work with a dedicated team to support recovery and help patients regain independence, administering medications, wound care, and conducting assessments in patients' homes.

You will develop individualized care plans with physicians and other professionals, monitor patient progress, educate families, and ensure accurate documentation.

Qualifications

• Nursing degree or diploma from an accredited program.
• Valid RN license in the state.
• CPR certification current.
• Experience collaborating with healthcare professionals.
• Driver's license and reliable transportation.
• Med-surg/ICU/ER/acute care experience preferred.

Responsibilities

• Provide skilled nursing care in patients' homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes promptly.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records and coordinate care with the medical team.
• Supervise Home Health Aides and ensure proper documentation.
