CenterWell Home Health in El Paso, TX is seeking a Home Health Physical Therapist Assistant to administer the physical therapy plan in patients' homes under the supervision of a Physical Therapist. You will help patients regain strength, mobility, and independence while delivering patient‑centered care.

Must hold a current Physical Therapy license, CPR, a valid driver's license with reliable transportation, and be fluent in English.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes.
• Read, write, and speak fluent English.

Responsabilités

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex active and passive manual therapeutic exercises and modalities.
• Document client records and submit clinical documentation on time weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist/Branch Director of equipment failures or changes.
• Apply infection control and universal precautions in patient care.
• Use skills to make an impact.
