Job summary

Caretender, part of LHC Group and aligned with UnitedHealth Group, seeks a Physical Therapist Assistant for home-based therapy services. You will implement treatment plans, guide patients through exercise programs, and educate families to support well-being under PT supervision.

The role emphasizes adherence to state laws, compassionate patient care, and collaboration with a multidisciplinary team to optimize outcomes.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to dependable vehicle, or public transportation.
• Ability to work with diverse patient populations.

Responsibilities

• Follow state-specific laws governing home care physical therapy and the supervising PT's plan of care.
• Instructs and assists patients with active and passive exercises, gait training, ADLs, and prosthetic training.
• Observe and report patient responses and coordinate care with the healthcare team.
• Educate patients, families, and caregivers on therapy techniques within scope of PT guidance.
