Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to serve patients in their homes under the supervision of the Clinical Manager. You will assess function, plan treatments, and document progress to support physician plans and family communications.

The role requires an active physical therapy license, CPR certification, and experience collaborating with healthcare teams. Schedule includes 40 hours weekly, with pay range reflecting geographic location and experience.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with physician plans and patient goals.
• Provide PTAs and Home Health Aides with clear care instructions and supervise documentation.
• Document patient observations, interventions, and evaluations promptly.
