Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home therapy, assess progress, and coordinate care with physicians and families. You will plan individualized rehabilitation programs and oversee aides to ensure high-quality documentation and safety.

The role emphasizes teamwork, patient-centered care, and adherence to physician plans, with a structured schedule and a strong focus on functional improvement.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist (preferred)

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
