South Arkansas Home Health, part of the LHC Group within the Optum family, seeks an Occupational Therapist for home health in Mount Holly, AR. You will assess functional status, ADLs, home safety, and adaptive equipment, coordinating with physicians and a care team to improve patient outcomes.

Formación

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to dependable vehicle.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsabilidades

• Provides service within the scope of practice as defined by state laws and in accordance with the plan of care.
• Evaluates patient’s functional status and CMS documentation needs; consults with physician on therapy plan of care.
• Observes, records, and reports patient response to treatment and condition changes to nurse supervisor and physician.
• Conducts patient assessments evaluating function using relevant tests and productivity targets.
