Job summary

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Valid drivers license and access to reliable transportation.

Responsibilities

• Assess and evaluate patient care needs related to functional status and ADLs.
• Collaborate with physicians and health care team on therapy plan of care.
• Document patient progress and report changes to nurse and physician.
• Perform home assessments and recommend adaptive equipment.
