CenterWell Home Health, a Humana Inc company, is seeking an experienced LPN to provide personalized nursing care in patients’ homes under an RN’s supervision. You will implement care plans, educate families, and document care and outcomes to support recovery and independence.

The role requires a valid nursing license, CPR certification, and at least two years of LPN/LVN experience in a clinical setting, plus a valid driver's license and reliable transportation.

Qualificações

• Current nursing license in the practicing state.
• Two years experience as an LPN/LVN in a clinical setting.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.

Responsabilidades

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.

Conhecimentos

LPN/LVN license
