Job summary

The Home for Little Wanderers is seeking a Family Partner to help caregivers navigate child-serving systems and improve family outcomes. This role involves home and community visits, coaching families, and collaborating with a care planning team.

Candidates should have caregiver experience and a background in human services; a valid driver’s license and access to a personal vehicle are required. Strong communication and documentation skills are essential.

Qualifications

• High school diploma or GED required; 2+ years experience with youth/children preferred.
• Associate’s or Bachelor’s degree in human services or related field with related experience.
• Fluency in a second language such as Spanish, Portuguese, or Haitian Creole preferred but not required.

Responsibilities

• Provide guidance, coaching, education, and support to caregivers throughout the care planning process.
• Work one-on-one with families to implement goals in the Individual Care Plan.
• Attend staff meetings and supervision; participate in clinical trainings and safety practices.
• Complete daily progress notes per MassHealth requirements.
• Enter billing and maintain caseload as assigned.
