Qualifications

• Must be 18 years of age or over.
• Prefer experience with individuals with disabilities.
• Home Health Aid/CAN credential a plus.

Responsibilities

• Implement Individual Support Plans for individuals served.
• Provide training in life skills and independent living.
• Administer medication as prescribed.
• Provide transportation for individuals served.

Skills

Working knowledge of Person Centered Planning

Oral/written communication skills

Ability to learn computer software applications

Cooperative attitude

Ability to maintain confidentiality

Education

High school diploma or GED

Prefer 6 months experience in human services
