Job summary

Qualifications

• Knowledge of community resources including social services agencies and crisis intervention providers.
• Understanding of processes, rules and regulations of various services and funding agencies for eligibility and maintenance.
• Familiarity with HIPAA regulations.

Responsibilities

• Assess and complete initial review processes including participant motivation, strengths, barriers and resources.
• Complete paperwork related to services provided.
• Help participants identify and access options through the program and local community.
• Develop and implement action plans with goals, steps, timelines and responsibilities.
• Provide ongoing support to participants and ensure program participation and compliance.
• Advocate for clients in referrals and contacts with other departments/agencies/companies.
• Maintain client progress documentation and program notes per QA plan.
• Report program progress per funding guidelines.
