Qualifications

• Assist people with disabilities to live in their own homes and participate in their community.
• Teach and coach daily living skills and personal care.
• Follow CSI policies, procedures, laws and regulations.

Responsibilities

• Support and empower each person to direct his or her own life.
• Develop and maintain positive relationships with persons supported, interacting positively and respectfully.
• Provide opportunities for learning new skills and life experiences.
• Teach and role-model appropriate social behavior.
• Assist in developing and implementing each person’s vision statement and support plans, including personal growth and daily living activities.
• Support each person’s health by aiding medical appointments and communicating with healthcare professionals.
• Help ensure individuals exercise their rights and maintain a safe, clean home.
• Maintain required certifications and trainings and complete documentation in a timely manner.
• Able to work 12 hour shifts and adhere to CSI policies for safety.
• Perform other duties as assigned.
