You will help clients engage in the community, express needs, and work toward person-centered goals with compassion.

Responsibilities include promoting independence, coordinating with teams, ensuring safety, assisting with daily living tasks, and transporting clients as needed.

Qualifications

• High School Diploma or equivalent is required.
• Compassionate and patient demeanor is essential.
• Strong communication and interpersonal skills are necessary.
• Ability to foster independence while providing required support.
• Knowledge of disability rights and community integration principles is preferred.
• Ability to maintain accurate records and follow regulatory guidelines.

Responsibilities

• Advocate for individuals’ rights and assist with personal care.
• Promote health and safety and support wellness initiatives.
• Encourage self-advocacy and independence.
• Facilitate community engagement and accompany individuals to activities.
• Provide transportation for appointments and activities.
