Job summary

Qualifications

• BA/BS degree or four years of equivalent education and/or experience in human services.
• Must pass KBI, DCF child & adult abuse checks, motor vehicle screen, and background checks.
• If the position requires the use of a personal vehicle, reliable insured transportation is required.

Responsibilities

• Assess employment needs with patients and develop personalized employment plans.
• Provide ongoing support and advocacy for employment goals.
• Collaborate with a multidisciplinary team to coordinate resources and opportunities.
• Conduct vocational assessments of skills, interests, and goals.
• Develop and implement individualized employment plans incorporating strengths and barriers.
• Provide job search assistance, resume development, and interview preparation.
• Coordinate with community partners to identify and secure employment opportunities.
• Advocate for reasonable accommodations and workplace supports as needed.
• Offer job coaching and on-the-job support for successful workforce integration.
• Maintain accurate documentation in the EHR and progress notes.
• Participate in team meetings and treatment planning for patients.
