Job summary

Spring Health in Las Vegas is seeking a Community Care Advocate to connect employees with community and company resources, addressing social determinants of health. This onsite role supports referrals for work-life, financial, legal, and basic needs, and guides employees through mental health resources and benefits.

You will document outcomes, collaborate with care teams, and promote services to employees in a fast-paced environment.

Qualifications

• Bachelor’s degree in a related field.
• Experience with diverse populations within social work.
• Ability to connect individuals to resources locally and beyond.
• Excellent listening and written/verbal communication.
• Comfort using technology in a tech-enabled environment.

Responsibilities

• Onsite role, Monday–Friday, 8:30–5:00 in zip codes 89119 and 89014.
• Identify referrals for work-life, financial, legal, and basic needs support.
• Link employees to local and national resources via referrals and consultations.
• Collaborate with internal care teams for emergency funding and referrals.
• Provide guidance on navigating mental health resources and benefits.
• Document metrics and outcomes for referrals and interventions.
• Promote the community care advocate service to employees.
