Qualifications

• Licensure as LPN or RN required.
• Caring approach and willingness to care for residents and the team.
• Ability to follow protocols and document findings accurately.

Responsibilities

• Conduct assessments, follow protocol, report changes, and document findings.
• Complete admission duties: assessment, MAR, TAR, POS, chart setup, orders, and meds.
• Manage discharges: documentation, notify physician/guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and dressings.
• Lead CNAs and CMTs to ensure policy compliance.
• Respond effectively to emergency situations.
