Job summary

Castleview Hospital in Price, Utah is seeking an LPN/LVN to join our nursing team. Under RN supervision, you will collect patient data, assist with plans of care, and perform care activities including wound care and medication administration.

You will support daily living activities, monitor patient status, and document care while advocating for patient well‑being. Successful candidates hold an approved practical nursing credential and UT licensure.

Qualifications

• Graduate of an approved practical nursing program.
• Current LPN/PN license to practice in UT or compact UT endorsement.
• BCLS certification within 30 days of hire/transfer.
• Knowledge of ICD-9, ICD-10, CPT codes, insurance benefits and healthcare background.

Responsibilities

• Under RN direction, contribute to patient assessment by collecting data for RN analysis.
• Perform plan of care interventions including specimen collection and wound care; may administer medications.
• Assist RN with development of plan of care.
• Assist patients with activities of daily living: hygiene, elimination, nutrition, ambulation.
• Monitor, document, and communicate patient status and responses to interventions.
• Document patient care provided and advocate for patient well-being.
• Schedule appointments and procedures as ordered by provider.
• Obtain insurance re-authorizations for testing and scheduled procedures.
