Qualifications

• High school diploma or equivalent is required.
• Six months or more experience with the specific population is preferred.
• CPR and de-escalation and restraint certification required (training available upon hire).
• First aid may be required by state or facility requirements.

Responsibilities

• Provide personal care services to patients under clinical or nursing direction.
• Demonstrate a positive, empathetic and professional attitude towards customers at all times.
• Conduct safety checks and ensure supervision is conducted at 15 minute intervals as needed.
• Document timely, accurate and appropriate clinical information in the patient’s medical record.
• Assist in providing a safe, secure and comfortable environment for patients, family and staff.
• Interact with patients, observe behaviors and communicate findings to nursing staff.
• Assist patients with activities of daily living including toileting, bathing, dressing, grooming and meals.
• May provide transportation for patients or coordinate transportation with staff.
