St. Luke's Health – The Woodlands Hospital is seeking an Emergency Room Clinical Pharmacist to support safe prescribing and dispensing. You will review prescriptions for interactions, dosing, allergies and other concerns, ensuring a high standard of patient care.

The role requires a PharmD or PharmD-equivalent, a TX pharmacist license, and CPR/BLS certification. You will work closely with physicians and nurses to optimize patient outcomes in a busy ER setting.

Qualifications

  • Bachelor of Science Pharmacy required at hire.
  • Doctor of Pharmacy required at hire.
  • Licensed Pharmacist in TX at hire; applicable state license required.
  • Basic Life Support (CPR) certification required at hire.

Responsibilities

  • Review every prescription in queue for safety and appropriateness.
  • Enter and verify orders into the computer system; label and dispense medications accurately.
  • Verify medications prepared by others and communicate drug information to patients and health professionals.
  • Clarify unclear orders with the prescribing provider to ensure safe therapy.
  • Maintain knowledge base to provide clinical services across all ages.
  • Remain proficient with automated dispensing and information systems.

Where You'll Work

St. Luke's Health - The Woodlands Hospital is a 245 bed facility serving North Harris and Montgomery counties. The hospital has been recognized as a Best Hospital by U.S. News & World Report, and is proud to serve as the clinical home for the Dan L Duncan Cancer Center. The hospital was awarded an "A" hospital safety grade by The Leapfrog Group in both 2023 and 2025. Clinical services provided include cardiovascular services, diagnostic imaging, women’s services (digital mammography and bone density studies), neurosciences, pediatric care (Newborn and Level III nurseries), surgery, pathology and pulmonary services, and sleep disorder services.

Job Summary and Responsibilities

As our Emergency Room Clinical Pharmacist, you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury.

Every day, you’ll build trust in these interactions by reviewing every prescription in your queue. We never rely entirely on the computer to check for safety; we always use an expert for the final check. That’s you. You’ll review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual in your opinion.

To excel in this role, you must enjoy learning new concepts and researching information in real‑time to provide solutions for our patients. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.

  • Demonstrates ability to document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure.
  • Interprets medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness. Accurately enters / verifies the order or prescription into the computer system, labels, and dispenses medications in a timely manner.
  • Verifies and checks medications prepared by other pharmacy personnel.
  • Communicates drug information to patients and health professionals. Resolves drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes. Unclear orders written by a licensed provider are clarified with the licensed provider. Assists the licensed provider in appropriate drug selection and therapeutic regimens.
  • Achieves and maintains an adequate basic knowledge base to provide clinical services for all age groups served. Provides basic clinical services as required by policy and procedure.
  • Maintains a working knowledge of automated systems.
