Responsibilities

• Assess patient status via interview, observation, and physical assessment.
• Develop an individualized care plan with patient/family and care team.
• Implement planned interventions to achieve outcomes.
• Document patient progress and update care plan as needed.
• Collaborate with healthcare team from admission to discharge to meet outcomes.
• Ensure patient safety and adherence to hospital policies.
