Job summary

Elevance Health is seeking an Emergency Placement Support Specialist to serve in North Carolina. The role is field-based, requiring residency in NC and travel to client sites or designated locations, with occasional meetings at the office.

Qualifications

  • MS/MA in social work or nursing degree with clinical experience.
  • Active license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist per state law.

Responsibilities

  • Manage overall healthcare costs via integrated case management.
  • Conduct assessments to identify needs.
  • Develop care plans addressing objectives identified during assessment.
  • Support member access to quality and cost-effective care and modify plans as needed.
  • Coordinate with internal and external resources to meet identified needs of the member for integrated whole-person care.
  • Coordinate social determinants of health to meet member needs and incorporate into care planning.
  • Work closely with state agencies.
  • Maintain knowledge of the philosophy of care and a spectrum of community-based services.
  • Build partnerships with designated populations addressing cultural and linguistic needs.
  • Evaluate health needs and identify applicable services and resources with members and families.
  • Provide information including education, medication reconciliation, and follow-up care.

Job description

Emergency Placement Support Specialist

North Carolina residency is required. This field‑based role will primarily be performed in the field, traveling to client sites or designated locations as needed, with occasional office attendance for meetings or training.

Travel within the assigned DSS Region is required, and candidates must reside within a commuting distance of an office to meet hybrid/virtual work policy.

Responsibilities
  • Perform case management telephonically and/or by home visits within the scope of licensure for special programs, such as Foster Care.
  • Manage overall healthcare costs for the designated population via integrated (physical health/behavioral health) case management and whole person’s health.
  • Conduct assessments to identify individual needs.
  • Develop comprehensive care plans to address objectives and goals identified during assessment.
  • Support member access to appropriate quality and cost‑effective care and modify plans as needed.
  • Coordinate with internal and external resources to meet identified needs of the member in terms of integrated (physical and behavioral) whole‑person care.
  • Coordinate social determinants of health to meet the needs of the members and incorporate them into care planning.
  • Work closely with various state agencies.
  • Maintain knowledge of the philosophy system of care; a spectrum of effective, community‑based services and support for those with or at risk for mental health or other challenges and their families.
  • Build meaningful partnerships with designated populations and their families, addressing cultural and linguistic needs.
  • Evaluate health needs and identify applicable services and resources in conjunction with members and their families.
  • Provide important information, including patient education, medication reconciliation, and identification of community resources, and assist with arrangement of follow‑up care.
Minimum Requirements
  • MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing with at least 3 years of clinical experience in social work counseling and a broad range of experience with complex psychiatric and substance abuse treatment; or any equivalent combination of education and experience.
  • Active, current, and valid license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist, as applicable by state law and scope of practice.
Preferred Skills, Capabilities and Experiences
  • Experience working with specialty populations and with complex psychiatric/substance abuse and/or medical disorders.
  • Ability to travel within the assigned DSS Region.
